First of all, stop giving your brain what it wants. When you supposedly relax or take a break your mind associates the act with a "feel good" feeling and releases toxins, you're stimulating your brain cells to react positively to breaks. I'm sure that a lot of us don't go to work with the mindset of "can't wait to finally work again, it's been a while" because we associated the act with a sense of angst. It's in our best interest to change this mentality, it can stop procrastination forever and I'm working on it everyday. You should too, it's the first step. Trick your mind into thinking your boring tasks are fun to do.
Secondly, let's stop worrying about all the nonsense. You most likely have a roof over your head and living healthily. Even if you're not, just the sheer fact that your heart beats right now gives you the opportunity to do great things. You woke up with a new chance today, don't take it for given. Do whatever you need to achieve your goals, if you put them off you're only prolonging the "pain". Start doing what you're supposed to right now, give your future self some breathing room and also a chance to create better things instead of being stuck in the past "cleaning up" after your old self.

A sense of "enlightment" came to me when I realized that I'm special. If Mark Zuckerberg can build a giant like Facebook why can't I do the same? He has the same number of hours in the day and the same brain as I do, why can't I use my time wisely and create world-changing things too? When I started to think like that I began to get motivated randomly throughout the day, my daily routine of just 4 hours of work quickly turned into 6-8 hours constantly and even 10 when I was determined to finish my tasks. Realize that every single person in this world is special, including you, and you have the power within to be as special as the greatest minds of the world. Don't forget that.
To sum things up, I guess I'm trying to say that you will not achieve your full potential if you're not focusing on what's important. Everyone has unlimited power inside them, it all comes down to how we use our minds to guide ourselves throughout life. We can either choose to relax or work hard, it's up to us to decide what's the most important.
