Simply download the app from the Google Play Store. Not sure if it's available for Apple devices though. Then you login with your Google and answer questions. Make sure not to lie though because they send questions to test you

2 months! The $0.14 surveys were where I was asked for a receipt (but I never keep receipts). You earn more the more places you visit.
